Are you watching Doctor X: Mafia in White on Netflix (NFLX) right now?
This recent hit medical drama has quietly racked up over 7 million views globally in a single week!
Recent headlines have focused on softening view hours, but those metrics primarily refer to the mature U.S. market. While that’s a valid consideration, it is hardly surprising for a saturated region.
The broader picture reveals massive, untapped potential across international markets.
I’m not dwelling on Netflix today, though. Instead, I’m diving into Intuitive Surgical (ISRG)—a company driven by a strikingly similar, yet even stronger, global expansion narrative.
And honestly? I’m sure the surgeons in that drama would love to have a couple of da Vinci systems in their operating theatres!
Strong Execution and Overcoming Slowing U.S. Growth
Intuitive continues to demonstrate operational strength this quarter. Revenue grew by a strong 22% year-on-year (YOY) to US$3.0 billion, while Non-GAAP EPS surged 25% to US$3.00 over the same period.
Despite beating analysts’ estimates, the market initially sold down the stock due to fears surrounding a moderating U.S. procedure growth rate and forward guidance indicating slightly lower Instrument & Accessories (I&A) revenue per procedure.
But just like with Netflix, focusing on maturing domestic growth misses the bigger picture: a premium business that systematically innovates to unlock new addressable markets, backed by multi-year global growth potential.
U.S. Procedure Growth: Maturation and New Runways
Let’s address the market’s main concern head-on: U.S. da Vinci procedure growth moderated to 10% YOY this quarter—its slowest pace since the pandemic.
It is understandably a sore point, given that the U.S. accounts for roughly two-thirds of Intuitive’s total revenue.
However, with an installed base exceeding 6,800 systems across U.S. hospitals, maintaining hyper-growth on such a massive baseline is mathematically unsustainable.
It is also vital not to mistake a lower growth rate for a loss of market share. Intuitive isn’t losing ground to competitors; it continues to actively capture market share, just at a steady, mature pace.
Crucially, management anticipated this natural maturation years ago and built alternative growth runways to transition the business seamlessly into its next phase:
- Unlocking High-Volume Benign Procedures: Backed by decades of clinical data, Intuitive’s deliberate strategy to lower I&A costs per case makes robotic surgery cost-competitive against traditional laparoscopy, opening up a massive, untapped market in routine hernias and gallbladder removals.
- Ambulatory Surgery Centers (ASCs) & Extended Use Programs: By expanding system placements into outpatient ASCs and extending instrument useful life spans, Intuitive is lowering the total cost of ownership for hospitals.
Far from being stuck at 10%, I wouldn’t be surprised to see a fresh surge in U.S. procedure volumes as these high-volume benign procedures gain critical mass.
The Longer Horizon Growth: Building New Terminals for Global Scaling
Intuitive Surgical is a great long-term compounder not simply because of its dominant core platform, but because of its strategic depth.
Management consistently executes in the present while positioning its next growth engines to take over as core markets mature.
If lowering I&A costs to capture routine benign procedures is akin to extending the U.S. growth runway, the following are three major growth terminals over the next decade:
- Terminal 1: The Outside U.S. (OUS) Expansion Engine
- Terminal 2: The Ion Platform (Robotic Pulmonology)
- Terminal 3: Unlocking High-Acuity Cardiac Surgery
Terminal 1: The OUS International Expansion Engine

While domestic procedure growth has moderated this year, Outside the U.S. (OUS) procedure volume continues to compound at approximately 20% YOY, fueled by strong adoption across Europe, Japan, and high-growth emerging markets like India.
The international installed base is growing rapidly, leaving an enormous runway ahead as adoption curves across Asia-Pacific and Europe track behind the mature U.S. market.
Terminal 2: The Ion Platform (Lung Diagnostics & Beyond)
Intuitive isn’t just expanding geographically; it is diversifying beyond traditional abdominal surgery into robotic endoluminal procedures.
The Ion endoluminal system—designed for minimally invasive lung biopsy—proves that Intuitive can successfully build, scale, and dominate entirely new clinical categories.
Granted, Ion’s revenue contribution remains modest relative to the da Vinci core today. However, given its hyper-growth trajectory (31% YOY procedure growth in the latest quarter), it is systematically scaling into a significant revenue engine.
To support expected long-term demand across Asia and shorten the time between lung cancer detection and treatment globally, Intuitive recently announced plans for a dedicated manufacturing facility in Penang, Malaysia, slated to begin operations in 2028.
Finally, while Ion is transforming early lung cancer detection and diagnosis today, its ultimate long-term catalyst is to allow doctors to diagnose and treat lesions in the exact same procedure.
Terminal 3: da Vinci 5 Unlocks Cardiac Surgery
Each innovation opens up new opportunities. The immense 10,000x computing power and micro-wristed precision of the da Vinci 5 (dV5) allow Intuitive to re-enter high-acuity cardiac surgery.
You can imagine the excitement among practitioners following the U.S. FDA clearance in early 2026 and subsequent European CE Mark approval later this year.
Finally, surgeons have a minimally invasive approach that replaces the trauma of open-chest sternotomies—resulting in less physical trauma, fewer post-operative complications, and a vastly accelerated recovery for patients.
Entering a Whole New Market
In many ways, this feels like when Intuitive first introduced robotic surgery to urology and gynecology two decades ago.
While procedure volumes in cardiac surgery are smaller than in broad general surgery, cardiac operations carry premium hospital economics and high clinical prestige.
However, don’t expect immediate top- and bottom-line accretion.
Unlike routine general surgery, robotic cardiac procedures require training entire operating room cohorts—including cardiac surgeons, anesthesiologists, perfusionists, and specialized nurses.
This team-based learning curve creates a natural adoption ramp in the near term, but over time, it will systematically unlock a vital new growth driver for the next decade.
My Action: Preparing to Add More

Looking back at my transaction history earlier this year, I am glad that my decision to increase my stake during periods of market pressure is already paying off.
However, even at last Friday’s closing price of US$452, I do not consider the stock expensive.
Assuming full-year FY 2026 Non-GAAP EPS comes in around US$11, Intuitive will be trading at a forward P/E of ~41x. This sits comfortably below its historical valuation multiples of above 60x, representing a very reasonable PEG ratio of approximately 1.6x.
If Intuitive maintains its current growth trajectory, it stands a strong chance of doubling its non-GAAP EPS over the next three to five years, making it an exceptional long-term compounder.
Having added earlier, I am in no hurry to aggressively chase the stock at current levels, but I am fully prepared to add to my position if the market decides to throw a tantrum again.
In the mean time, I will continue to chase Doctor X: Mafia in White.
