Imagine you run a sales team and set a target to improve sales by 20% to hit $100,000 by year-end.
Your team absolutely crushed the first half, booking $55,000 by mid-year.
When asked for a revised projection, you prudently decide to stick to your original target to give the team a safety buffer against unexpected macroeconomic headwinds in the second half.
Instead of congratulating you for a job well done, your boss claims that you are “underperforming” and slashes your sales bonus by 15%.
Does this make any sense to you?
Not to me, but this is the exact brand of broken logic Wall Street just applied to Intuitive Surgical (ISRG) following its 2Q 2026 earnings release.

The Absurdity of Wall Street’s Reaction
For the quarter, Intuitive grew its revenue by 19% YOY to US$2.89 billion and Non-GAAP diluted EPS surged 28% YOY to US$2.80, well above consensus estimates.
Yet, despite this massive beat, the stock plunged by more than 14% the next day, closing at around US$345.
Why?
Simply because management decided to be prudent and hold their full-year guidance of worldwide da Vinci procedure growth at 13.5% to 15.5%, landing somewhere near the midpoint of 14.5%.
Management shared during the earnings call that while the first half of the year ran closer to 15%, it expects some moderation in the second half because of tougher comparisons and seasonal effects.
This implied 1% lower growth guidance in the second half is enough to set off a panic.
Analysts zoom in to the slowdown in U.S. procedures, which dipped from 14% growth in the first quarter to 12% in the second.
The culprit? A temporary macroeconomic speed bump: patients deferring surgeries after losing health insurance due to the expiration of enhanced Affordable Care Act (ACA) tax credits.
But is this a structural breakdown, or just temporary noise?
When you zoom out, it becomes clear that Wall Street is missing the forest for the trees.
The Long-term Growth Stays Intact

To understand why this 14% drop is an overreaction, you have to separate short-term macroeconomic friction from the structural health of the business.
Let’s delve into the primary pillars driving Intuitive’s multi-year runway.
The U.S. Slowdown: Speed Bump Not Roadblock
The friction is real.
Losing health insurance or being forced to downgrade to a plan with lower coverage means paying significantly more out-of-pocket cash for medical care.
When faced with high deductibles, deciding to live with the discomfort and defer a benign elective surgery sounds perfectly human.
However, a patient cannot defer a necessary surgery forever.
Whether navigating new insurance plans or saving up for deductibles, these patients simply become backlogged demand that will eventually hit the operating room.
In fact, core demand is far from dead — U.S. after-hours procedure volumes actually exploded by 26%, proving that maxed-out hospitals are working their machines late into the night just to clear the administrative traffic jam.
Ambulatory Surgery Centers (ASCs): Making Inroads
Meanwhile, Intuitive isn’t just waiting for these hospital bottlenecks to clear; they are actively expanding outside traditional hospital walls into ASCs where day surgeries are carried out.
By factory-refurbishing their older machines, Intuitive is now able to offer the da Vinci XiR at a highly affordable, discounted price to these leaner outpatient centers.
This hardware play will be further supercharged by the introduction of the next phase of their Extended Use Program (EUP) in the first half of 2027.
By stretching the lifespan of core instruments, the EUP will drastically slash per-procedure costs, making a da Vinci surgery highly competitive with traditional, cheap keyhole surgery (laparoscopy).
All these initiatives take time to mature, but it is clear that the U.S. growth runway is not coming to an abrupt end.
Outside the U.S. (OUS): Driving the Growth
Granted, the U.S. still contributes the bulk of total revenue (approximately two-thirds). However, as domestic growth experiences a temporary digestion phase, a resilient international runway helps mitigate the impact.
The Real Catalyst: What makes this OUS runway even more compelling is that this massive volume growth has been achieved before Intuitive’s next-generation hardware has even hit the ground internationally.
Over the past two years, Intuitive has quietly secured critical regulatory approvals for the da Vinci 5 (DV5) across major global markets, including Canada, Europe, Japan, South Korea, and most recently, India.
As these regions transition from the regulatory phase into full commercialisation, this international footprint is set to provide a powerful, multi-year tailwind for the business.
Expanding Total Addressable Market (TAM): Beyond Current Surgeries
It is short-sighted to just focus on quarterly numbers and miss Intuitive’s long-term strategies to expand its TAM.
The latest DV5 with next-generation force-feedback technology, is not only getting positive feedback from early-adopting surgeons, but has paved the way for FDA clearance in thoracoscopic cardiac procedures.
By breaking into intricate intracardiac surgeries, Intuitive is unlocking an entirely new high-value clinical pipeline.
The market also forgets the recent approvals they have gotten for da Vinci SP (Single Port), enabled by the rollout of the hyper-flexible SureForm 45 stapler:
- General Surgery: Allows the SP system to perform mainstream, high-volume abdominal procedures like hernia repairs and gallbladder removals through a single 2.5 cm incision.
- Oncology: Enables complex cancer interventions through a single hidden incision, notably expanding into transanal colorectal oncology and nipple-sparing breast cancer mastectomies.
Finally, Ion, which deals with robotic-assisted endoluminal lung biopsies, continues to gain tremendous traction.
Ion procedures surged an incredible 36% YOY in 2Q2026, quietly establishing itself as a standalone multi-year compounding engine that captures recurring revenue before a patient ever sets foot in a standard operating room.
I’m not sure about you. But to me, that’s multi-year of pipelines to grow into, actively widening a multi-generational competitive moat.
Intuitive Is Just Too Expensive?

I don’t deny that.
Intuitive has always traded at a premium, with its long-term historical average P/E exceeding 60x.
Even though I love the business, it’s never easy for me to push the buy button at those lofty valuations.
But with its price plummeted by a whopping 39% this year, is it finally more palatable?
Projected FY2026 EPS Baseline
While management does not provide specific EPS guidance, if we model the midpoint of their operational targets — 14.5% worldwide procedure growth and a 68.5% non-GAAP gross margin — FY2026 Non-GAAP EPS lands right around US$10.60.
With Intuitive already locking in US$5.30 in Non-GAAP EPS for the first half of the year, a full-year result of US$10.60 implies zero sequential growth in the second half.
That is highly unlikely.
The fourth quarter is historically a massive seasonal spike for this business, which means there is a very good chance that full-year EPS could comfortably clear our baseline and even hit US$11.00 for the year.
The Forward P/E and PEG Reality
Even if you play it safe and stick to the conservative US$10.60 baseline, the current share price of US$345 compresses the forward metrics into incredibly rare territory:
- Forward P/E: The multiple drops to ~33x.
- The PEG Ratio: Pair that 33x forward multiple against its 19% projected YOY EPS growth, the forward PEG is just around 1.7.
It’s rare to see a valuation window like this open up for Intuitive.
That said, a shrinking multiple doesn’t automatically mean a stock is a bargain.
My experiences with Lululemon (LULU) and The Trade Desk (TTD) taught me that firsthand. I bought into their valuation drops, but the businesses couldn’t reverse their slowing growth and the stocks never recovered.
There is a huge difference between a stock getting cheaper and a stock offering real value.
Having invested in the stock since early 2020, my conviction with Intuitive is fundamentally higher, though. Its strong execution track record, robust balance sheet, and deeper competitive moat set it apart.
Hence, I took Friday’s opportunity to add to my position at US$359, bringing my average cost price to US$307.
In the short term, market fear will likely linger, but looking out a few years, the earnings engine should do the heavy lifting.
